How can a pre-existing condition affect insurance eligibility?

A pre-existing condition or history of medication, tests, hospital stays or surgery can affect underwriting for some health and accident insurance. Through pre-contract disclosure questions, the insurer checks specified health information over a stated period and decides eligibility and terms after assessing the risk.

Possible outcomes include:

  • acceptance on standard terms;
  • a request for further information or medical documents;
  • exclusion of a particular condition or part of the body;
  • different premium or coverage terms;
  • information about another product; or
  • refusal of the application.

It is therefore wrong to say that any medical history always prevents enrollment. It is also wrong to say that everyone will be accepted for a simplified-underwriting product.

An example from official information

The Financial Services Commission has published information about medical-expense insurance for consumers whose medical history makes standard medical-expense insurance difficult to obtain. Some insurers currently offer products for people with medical histories using simplified questions about recent tests, treatment, hospital stays, surgery and cancer history. They also state that enrollment may be restricted or declined under the insurer’s underwriting criteria.

This shows that separate products for people with medical histories exist. It does not guarantee acceptance for any particular condition. The periods covered by the questions, underwriting rules, out-of-pocket amounts, premiums and coverage differ by insurer and product.

The most important principle is to answer the insurer’s questions accurately. Respond to the precise scope of its official questions and document requests, and ask the insurer if a question is unclear. Even if an agent or another person says that something need not be mentioned, do not narrow the formal application question yourself or omit an important fact.

What to check before applying

  • What period of medical visits, medication and tests is covered by the questions?
  • Were you offered standard or simplified underwriting?
  • Is a particular condition or part of the body excluded?
  • How do the out-of-pocket amounts and premiums differ?
  • Are additional medical documents required?
  • Has the reason for refusal or conditional acceptance been explained?
