What happens if foreign residents do not pay NHIS contributions?

An unpaid NHIS bill does not simply disappear. Arrears can lead to collection action and, under the applicable rules, restrictions on insurance benefits. The exact effect depends on the insurance category, the arrears period and whether a statutory exception applies.

Current rules for foreign local-insured residents allow benefit restrictions after the prescribed arrears period. Some limited exceptions can apply, including rules for certain residence statuses and short arrears, so it is inaccurate to say that every missed payment immediately removes all cover. When overdue contributions are paid, the rules on restoring or recognising benefits should be confirmed with NHIS for the individual record.

Arrears can also matter in immigration administration. The Ministry of Justice states that NHIS arrears are checked for certain applications to extend stay. Full payment normally allows the ordinary extension period, while unpaid arrears can lead to a restricted extension, generally up to six months at a time. This is not the same as saying that every arrear automatically causes visa refusal or an entry ban.

If you are abroad or believe the bill is wrong, do not ignore it. Ask NHIS to confirm the period, eligibility category, amount, payment method and whether departure or a status change was recorded correctly.
